Broadcast and Post Production Professionls: a question about HDTV

Especially for those of you in the US I guess.

What are the favoured content distribution methods for HD materials at the moment, primarily for finished pre-recorded media (ads, promos,m programming, movies) but also between location and editing and around the post production chain?

I know what's out there in terms of tape, disk and other storage systems but I'm interested in knowing what's actually in use.

workflow: tape ----> digital capture files ---> dvd for distro.

Which codec on the DVD?

MPEG-2 , H.264, MXF?

QT wrapper, AVI, .mpg?

Anyone got any ideas for MAM solutions that work with both FCP and Premiere.
