Exactly when did smoking go completely beyond the pale?

Watching the local TV news about new initiatives banning cigarettes everywhere, the vox pops are quite extraordinarily hostile. "Good, they shouldn't have any right to spread their filthy pollution anywhere", etc. When was the major "tipping point" where it stopped being a debate and turned into a legitimate floodgate-opening for all this vitriol? Well, maybe it never really was an actual 'debate', but people used to be more polite about it.

i have asthma and smoking bothers me a lot, but i think you know its just one of those things people can feel morally superior and indignant about. its like the people who inhabit the office building where i work complaining about the smell of mcdonalds in the elevator-same attitude.

it's smoking at gigs in small clubs that bothers me the most (to make this more of an ILM topic). In London, it still looks like a long long way til there's smokefree gigs in clubs.
